6 - Getting almost all the way through a stealth mission and then press the wrong button (jumping up or firing at your foot etc.) and being insta-killed.  Especially in a co-op game :o(

My worse moments are either blowing myself up with a grenade or having the game glitch and i have to start all over earning more money to rebuy the same weapons
